Registration Requirements

We accept applicants for registration Monday – Friday, from 9:00 AM to 4:00 PM .

At time of registration, the following items are required: • Current Driver’s License (or official Florida ID)

Laws & Rules for Florida Nurses
Proof of Automobile Insurance
Social Security Card
Professional License (or certificate for HHA’s)
Current CPR card
Health Certificate within the last 6 months with current Negative PPD or Chest X-Ray
